New Patient Information
Our practice welcomes new patients every day and feels privileged to be available to so many referrals from our current patients and friends. Your first visit will be the longest
encounter because we want to spend time getting to know you and working on a
customized health plan for you. Sometimes due to the complexity of issues we
may need to do a follow up visit at a time that would be appropriate.
About the "New Patient Information" Form...

If you have been treated at some other facility and we are going to
discuss your problems, it would be best to forward your medical records
in advance so the doctor can be prepared for your visit. The "Release of Medical Records" form can be found at the end of the "New Patient Information" form. COMPLETE, DATE and SIGN, then fax OR mail to the
appropriate doctor, clinic, ER, and/or hospital.
All medical information is protected by HIPPA regulations (see
HIPPA section and fill out your acknowledgement form) to which we
strongly adhere. No medical information is ever divulged without your
written consent.

Financial Policy
We participate in most insurance plans and will file your insurance claim for reimbursement. On the other hand, it is your responsibility to know the details of your plan regarding referrals, co-payments and deductibles. Please bring your current insurance card and a photo ID on the day of your visit.
A co-payment is required at every visit with most plans, so kindly beprepared in advance. We accept cash, checks and most credit/debit cards.
Please call our billing/insurance department at 770 939-5102 for any
questions related to insurance issues. Payment in full is expected in
cases where there is no insurance. We will assist you with payment
plans and special arrangements like surgery charges upon request.
